Fever

hello all doctors..a 12yr old male child referred to me with c/o vomittings generalised body pains n fever for last 3days..non projectile vomit,soon after feeds..on repeated asking child said he had burning micturition also.. child is sick looking ,dry coated tongue n not even able to sit during examination..on repeated poking childs mother said dat child is not well for past 3mons..when he developed a febrile illness with falling blood counts and jaundice,, admitted at a higher centre n received blood transfusion..since then child had productive cough n low grade fever sometimes requiring admission..o/e pulse 110 normal vol,rr 50,spo2 82 without 02,maintainable wit 3lit02..but RS clear no adv.sounds..abd 4cmhepatomegaly+..,CVS nad..cns no meningeal signs n focal deficits..no tb contact..only child of parents..no high risk behaviour n child works as shepherd not going to school..kindly discuss the possibilities..n approach..reports attached..i have started child on larinate doxy n ceftriaxone..

4 y female with fever of 6 days and pain abdomen, sick looking. admitted, investigation ordered_ TLC 27700 , N- 91 %, WIDAL TO, TH both 1: 160. other inv- WNL. Started on inj crftriaxone and oral azithro. Fever continued high grade with fast breathing . Repeat counts 35000 , N- 71. Diagnosis Typhoid fever with sepsis. CXR done as shown. Large consolidation with pl.effusion. Added inj. vancomycin. Patient slightly better with improved activity and fever subsiding.

Sir. 3 years female high grade fever 9 days No history of cold cough Little weight loss please give advise patient admitted under Peadiatrician's advised bone marrow biopsy

25 yr old female, recurrent fever, high grade, 1 month apart recurrence , 3d episode. Fever with 104F reduces slightly on medication . No symptoms of uti, except increased frequency , blood n urine routine n cultures negative, urine has few bacteria , malaria negative, usg and n pelvis found right renal cortex cyst, ct kub cyst only, Dolo 650 n iv antibiotic was given for 5 days bd. Last day of course, b complex was started for oral ulcers n rashes macular was seen all over the body. Allegra n calamine lotion prescribed . Next day knee pain n pedal edema was noticed, with pain in the feet. For which aceclofenac tab was given for 10 days Was improving, but on exertion a day ago, pedal edema has started again with pain in the feet n difficulty in walking. Patient cannot walk climb stairs or get down, pain severe m sleep disturbed. What is the diagnosis m treatment now?

11yr female c/o fever since 1week with occasional pain in abdomen,h/o constipation occasionally..Investigations done typhoid fever ruled,usg abdomen shows mild to moderate asciitis,mild hepatomegaly ..kindly discuss the case
