Diagnosis of BPH

Early diagnosis of BPH is important because if left untreated it can lead to urinary tract infections, bladder or kidney damage, bladder stones and incontinence. BPH ie benign enlargement of prostate seen in elderly males is important to understand from the point of view of complications seen in untreated cases like LUTS stones or long standing chr renal disease Next benign can turn in malignant with monitoring of PSA we can save this difficult disease to treat as casteration will save further progress and give a quality life of more than 10yrs So far diagnosis of f BPH in day to day OPD is easy with detail history of urine flow retention and time taken to micturate Or just trained yourself for PR and locate palpate the prostate try to reac upper border if not reachable than it is BPH Now the point to differentiate BEP and BPH Very little difference is noted as line of treatment differs a lot BEP is mild hyperplesia presenting with frequency or nocturnal urea or urgency Back pressure is not competent for bladder to sustained these pts can be treated conservatively and surgery can be delayed I prefer silodosin dutcheride8mg od Rest of LUTS symptoms are treated as seen BPH is enlarged median lobe obstructating free flow of urine pt takes double of time to micturate and still not completely evacuted at times uses pressure to micturate USG suggest significant retention and hence frequency urgency and dribbling are mostly seen Pt loose control even cloth wetting is seen These pts need urgent surgery TURP to relieve Lastly BPH with increased PSA a and hematuria suggest malignant prostate and need to take action accordingly
